Dear residents and staff of Advantage of Southgate. I am writing to provide you with an update on Covid 19 protocols. Currently, we do not have any patients or staff who have tested positive or have been recently treated for Covid 19 here at Advantage of Southgate. As move to normalcy, we are now allowing visitors back into Advantage of Southgate. With this change, the following must be observed.



· All visitors & staff must check-in at the front door. Visitors and staff must fill out the Covid-19 screening tool. During this process staff and visitors will be givien a mask and required to clean their hands.

· Residents and visitors may visit without distancing and source control masks if alone in the resident’s room and the resident and visitor are fully vaccinated. If the visitor is not vaccinated, they can visit with a fully vaccinated resident alone in the resident’s room without distancing while wearing well-fitting source control masks. If a resident or visitor is not vaccinated, they will be required to wear a mask and maintain social distancing.

· Communal dining and activities may occur without physical distancing if all residents are vaccinated. If there is a resident in attendance who have not fully vaccinated, they will need to follow the social distancing guidelines outlined by the CDC

· If vaccination status cannot be determined, the safest practice is for all participants to follow all recommended infection prevention and control practices, including maintaining physical distancing and wearing source control.

With the new guidelines in place, we are asking all residents and staff to report any new issues with shortness of breath, elevated temperature, and cough to the charge nurse or the Director of Nursing. At this time, we are prepared to take care of all residents who believe they may have signs and symptoms of Covid 19. To prevent the spread of Covid-19 we are asking everyone to wash their hands, covering their cough, and use hand sanitizer before touching themselves or someone else. Can you believe it, it is almost March of 2021, wow does the time fly! Over the past year, Advantage living Center Southgate has done everything in its power to keep the residents of Advantage Living Center Southgate safe. We have invested in technology that has helped to keep everyone safe from Covid-19 and other viruses. Advantage Living Center Southgate has remained COVID-19 free for months. We continue to clean commonly touch areas often and to ensure no one enters the building without fill out our screening questions. All staff members and visitors entering the building must have a recent covid-19 test before they interact with any resident. Hello, I have some exciting news. We have diligently been working on a reopening plan for our building. As of 10/26/20 Advantage Living Center - Southgate will be open for in person visits. All visitors wishing to visit a loved one will have to make an appointment before being allowed into the building. Appointments can be made one of two ways. The first way is by calling the front desk and asking to speak with the Activities director Ann Marie. Ann Marie can set an appointment date and time for your visit. The other way is by logging onto our Facebook page and signing up by using the signup genius link posted at the top of our Facebook page.

Before entering to see your loved one you must do the following:

1. Have in hand a current negative Covid 19 test. This test must be within the last three days prior to your visit.

2. Have signed the Advantage Living Center -Southgate contract

3. Wear a mask and the appropriate PPE

4. Wash your hands and have your temperature checked at the front door.

Residents are limited to two visitors at a time. Each visit will be 30 minutes in length. During the visit you and your loved one must remain 6 feet apart. No food or drinks can be brought in and exchanged during the visit.

I would like to share some other good news with you as well. This week we have started in person dining. We are starting with lunch today and tomorrow. Starting November 2nd , we will be having lunch 7 days a week. My staff will round daily to ask you if you would like to attend lunch. We have also started back in person activities. Please speak with any member of the Activities team to find out more information about group activities.

Our outside drive-by visits have been a huge success. Please notify the Activity Department if you would like to schedule a visit outside or to see your loved one. This letter is to inform you that a vendor has test positive for Covid 19. This vendor did have direct access to the building and some patients. As you know, we take Covid 19 very seriously. The following addresses our mitigation strategies.

Dear residents and staff,

It is with great pleasure to announce that we are officially Covid-19 free! As of 6/19/20, we have discharged our last Covid-19 resident from our Covid unit. To date, we’ve had 38 total residents and staff diagnosed with Covid-19. I am happy to say that all 38 have made a full recovery!

As of 6/15/19 the State of Michigan has asked that we test all residents and staff for Covid-19 on a weekly basis, until further notice. This test will be conducted by the nursing department. All previously negative tested residents and staff must have another test completed. If you have previously tested positive for Covid-19 you do not need to patriciate in the ongoing testing. Testing for residents will be conducted on 6/23/20 & 6/24/20. Testing for staff will be conducted on 6/25/20, 6/26/20, 6/27/20 and 6/28/20. The test will be conducted in a private location and is free of charge. Vendors entering the facility will have to provide proof of their negative test or comply with the facility’s testing requirements. This will be completed on a weekly basis as well, unless otherwise directed by the State of Michigan.

I cannot thank everyone enough, for doing their part during this Covid crisis. Please continue to wear your mask and wash your hands frequently. Below are our mitigation strategies to support and promote safety for all our residents & staff.

Things are looking up. At this time the governor of MI has released the stay at home order. Currently, we are still under a non-visitor restriction until the end of June. As we progress back to normal, I want to remind everyone that the best way to prevent the spread of infection n or COVID 19 is by handwashing and wearing your mask while in public. We have officially gone one month without any new cases of Covid-19. Currently, we are providing care for 11 Covid 19 resident that are asymptomatic. I’d like to thank the dedicated housekeeping team here at Advantage Southgate for keeping us safe. As a reminder, we have weekly family visits outside. Please see our facebook page to sign up. We are excited to welcome back our residents from our sister facilities. Today we have 10 current asymptomatic COVID-19 patients. The 10 patients that are positive are not new COVID residents, they are the remaining residents from our original test date on 5/4/20. The remaining residents will continue to remain on our dedicated COVID unit. To date there has been a total 28 residents and staff who have received from COVID-19.
