1. Referral – the hospital or your Physician will make a referral and a nurse from the facility will be sure our team is equipped to meet your needs including:

• Therapy

• Respiratory illness

• Pain

• G tubes

• Catheters

• Diabetes       

• Cardiac conditions

• And many more

2. The Orchard’s Case Managers will then

a.    Do the leg work.

b.   We make the calls, contact the insurance company, and make the transportation arrangements. 

3. Complete the documents with an Orchard team member.
