Welcome to The Orchards Harper Woods

As a new resident, here are some things you can expect:

· Upon arrival, the nursing staff will be in to do a full assessment. They will review your hospital transfer records and medications. We will be making frequent checks on you to ensure your needs are being met.

· Your specific medications are ordered after your arrival. We will check on you to assess any issues with pain or if you have any other medical concerns that we can address.

· Therapy will be conducting an evaluation within 24 hours. Most residents will be receiving therapy five out of seven days.

· We will ask for your clothes at admission so that laundry can label and wash them at time of admission as they are coming from home.

· Social Work will set up a Care Conference with the Interdisciplinary Team to discuss transition home, whether your goal is to return home, to a new home or reside in long term care. Our Social Services team will assess your needs and coordinate this process with our discharge planner to ensure the required services you are set up prior to transitioning home.

· Individual leisure needs will be met by the team and customized to your interests. At this time, we’re offering Virtual Visits between you and your family members with email access, coordinated by our Life Enrichment department. We also encourage closed window visits. Families are welcomed to call the facility to request assistance with these visits prior to coming to the building for visit. We will coordinate with your family and friends to ensure you receive any items that are brought to the facility for you. Please limit any paper bags, if transporting items please use plastic bags.

· During this time, some appointment we will not be able to schedule. Following state guidelines from the Governor, only emergency appointments will be scheduled. Evaluation will be based on the individual’s needs.

· When residents are admitted to Skilled Nursing Facilities such as ours, throughout the nation right now, there is currently a 14-day period for all new admissions to our observation unit, to ensure your safety and the safety of the other residents and our employees.

· Our nursing staff will be wearing Personal Protective Equipment which includes face masks or face/eye shields and a protective gown.

· You will be staying in this room for 14 days and then, if you are not quite ready to go home, we will transfer you to another room for the remainder of your stay with us. While the staff caring for you at this time will still be wearing face masks, full PPE” .Your door will be closed per state guidelines. We will keep you informed of what the Centers for Disease Control and the State of Michigan is allowing us to do in regard to having you go out of your room for meals, therapy and activities. We will also keep you informed of any changes in visitation.
